Forks Community Hospital’s (FCH) commitment to exceptional, personalized care continues to grow with the recent achievements of Joanna Hofer, CHW, and Erika Loveless, CHW, both of whom have earned their Community Health Worker (CHW) certification. After completing a comprehensive Community Health Worker Training Program through the Washington State Department of Health, Joanna and Erika now bring new levels of care to the West End.
Community health workers are essential and trusted members of care teams and serve patients, families, and community members outside of the walls of a healthcare setting. “At FCH, we are always seeking ways to improve health outcomes, and that includes providing education, guidance on preventive care, support for managing chronic conditions, and, wherever possible, helping those we serve access essential services that contribute to wellness and better health,” said Heidi Anderson, Chief Executive Officer.

For Erika Loveless, CHW, the certification represents more than professional growth—it reflects a deeper connection to the community she serves.
“Becoming a Community Health Worker has been one of the most meaningful and impactful steps in my professional journey. The training not only strengthened my understanding of patient-centered care but also deepened my appreciation for the unique challenges and strengths within the communities we serve. It provided practical tools, cultural awareness, and a renewed sense of purpose that directly support my role as a Community Patient Resource Specialist.”

In her role, Erika works closely with inpatients to coordinate care as they prepare for discharge. She ensures that each patient leaves the hospital with a clear, supportive plan—one that addresses both medical and social needs and helps ease the transition into outpatient care.
Joanna Hofer, CHW, complements this work by helping patients access the broader network of healthcare and community resources. She partners with patients to identify barriers and connect them with services that improve their ability to receive care and enhance their quality of life.
