Requirements for Visitation at FCH Long-Term Care

Dear Guardians, POAs, Visitors, and Family Members,

Washington State Department of Health has directed Nursing Homes in Washington to begin the process of opening facilities for resident visitation.  Forks Community Hospital Long Term Care Unit has begun this process and at this time, visitation is not fully open, however, below you will find the process under which we may open our facility for safe resident visitation.

Because we have unvaccinated residents in our facility, we must follow the guidelines set forth by Washington State Department of Social and Health Services.

 

INDOOR VISITATION:

  1. Indoor visits may take place in the Patio Room if either the resident or the visitor has been fully vaccinated.
  2. You must call and make an appointment as the patio room can only safely accommodate up to four (4) people, one (1) resident and three (3) visitors. Only one resident may have visitation at a time in the patio room.   We must allow for social distancing during the visitation.  If the resident is fully vaccinated, they can choose to have close contact (including touch) with their visitor while wearing face masks.  Cloth masks or face masks must be worn; face shields are not allowed during this visitation.
  3. Visits will be on the hour and last for thirty (30) minutes. This allows for the room to be disinfected before the next visit. This will allow everyone time to use the room for visitation.
  4. You must be screened at the Screening Shed at Bogachiel Clinic prior to entering the patio room. It is open Monday – Friday.   After hours and on weekends, the LTC nursing staff will need to screen you prior to entry.
  5. Complete hand hygiene upon entry
  6. You must sign in upon entry to the patio room to include:
    1.  Name
    2. Phone number where you can be reached
    3. Answer the COVID-19 questions
    4. Acknowledge that you have reviewed the notice about unvaccinated visitors and residents.
  7. At no time may a visitor enter the hallway. If assistance is needed, please turn on the call light.
  8. Visitors with signs/symptoms of COVID-19 or those who have had close contact with someone with COVID-19 infection in the prior 14 days (regardless of the visitor’s vaccination status) will be denied entry.

 

OUTDOOR VISITATION:

  1. May occur during periods of appropriate weather if the resident is agreeable.
  2. If the resident and visitor are unvaccinated, this is the only visitation option available at this time.
  3. You must be screened at the Screening Shed at Bogachiel Clinic prior to entering the patio. It is open Monday – Friday.   After hours and on weekends, the LTC nursing staff will need to screen you prior to entry.
  4. You must call and make an appointment as the patio can only safely accommodate up to four (4) people, one (1) resident and three (3) visitors. We must allow for social distancing during the visitation.  If the resident is fully vaccinated, they can choose to have close contact (including touch) with their visitor while wearing face masks.  Cloth masks or face masks must be worn; face shields are not allowed during this visitation.
  5. Children must be able to adhere to the standards above, including hand hygiene and wearing of face masks if older than two (2) years of age.
  6. You must sign in upon entry to the patio to include:
    1. Name
    2. Phone number where you can be reached
    3. Answer the COVID-19 questions
    4. Acknowledge that you have reviewed the notice about unvaccinated visitors and residents.
  7. Visitors with signs/symptoms of COVID-19 or those who have had close contact with someone with COVID-19 infection in the prior fourteen (14) days (regardless of the visitor’s vaccination status) will be denied entry.

These guidelines have been put in place to keep our residents safe.  Thank you for being patient with us while we attempted to keep your loved one safe from COVID-19; we have remained COVID free for one (1) year.  Forks Long Term Care is one of only 64 nursing homes nationwide who have kept the Covid virus out (Government Accountability Office report, May 2021).  Please help us to continue to protect your Friends and Loved Ones.  We want you to enjoy your visitation as much as possible.
