Free COVID-19 Vaccination Information

COVID-19 Vaccinations are Available in forks Washington

Are you at least 12 years old and in need of a COVID-19 vaccine?

Bogachiel Clinic offers the Moderna vaccine to patients aged 12 years and older.
If you are aged 12-17, you will need to:

  • Bring a guardian to give vaccination consent
  • Bring this form, completed by your legal guardian
  • Have a legal guardian available by phone to give verbal consent.

Everyone Ages 12 and Older Can Get an Additional Dose

 

If you received Pfizer – BioNTech or Moderna

You should get an additional 3rd dose if you are:

  • Ages 12 years and older

When to get an additional dose:

At least 5 months after completing your primary COVID-19 vaccination series.

According to the CDC, any of the COVID-19 vaccines authorized in the United States may be used as your additional dose.

 

 

If you received Johnson and Johnson’s Janssen

You should get an additional dose if you are:

  • 18 years or older

When to get an additional dose:

At least 2 months after your shot.

According to the CDC, any of the COVID-19 vaccines authorized in the United States can be used for the additional dose.

4th Dose Information

At this time the CDC recommends a 4th dose for individuals that are moderately or severely immunocompromised and over the age of 11.

If you already have three doses of the Moderna or Pfizer COVID-19 vaccine or a combination of the two and would like the 4th dose, you must speak with your provider. At this time, 4th doses can only be ordered by a patient’s primary care provider and cannot be given at a nurse-only appointment.

 
If you had the Johnson and Johnson COVID-19 Vaccine and 1 additional dose of either the Moderna or Pfizer COVID-19 vaccine and would like an additional dose, you must see your provider. At this time, the 3rd dose with an initial Johnson and Johnson vaccine can only be ordered by a patient’s primary care provider and cannot be given at a nurse-only appointment.

The Health Department in Forks offers the Moderna and Pfizer COVID-19 vaccines on Thursdays to people 6 months of age or older.
