The Forks Community Hospital Patient and Family Advisory Council is dedicated to enhancing healthcare for all patients, with the patients at the center of all discussions.
Forks Community Hospital’s Patient and Family Advisory Council’s purpose is to improve the patient experience and the overall safety and quality for patients. This council will consist of hospital leadership staff, board, and community members who have either been a patient or have had a close family member as a patient.

Mission:
It is our mission to serve as an advisory resource to the Forks Community Hospital to provide excellence in patient-centered care.
Vision:
The Forks Community Hospital Patient and Family Advisory Council will be dedicated to enhancing healthcare for all patients, with the patient at the center of all decisions.
Purpose:
The Forks Community Hospital Patient and Family Advisory Council will work together to improve the quality and safety of care within the Hospital District. Together, we will have an active role in improving the patient and family experience by providing feedback from previous patient experiences and utilizing this to implement new and improve existing plans, activities, and programs related to patient-centered healthcare with the ultimate goal of improving hospital quality and safety.
Should you become a Patient and Family Advisor?
YES, if:
You or your family members were treated at FCH and you thought there were things that we could have done better at that time.
You have ideas to ensure that patients and their families are getting the best care possible.
You would like to help us to improve patient safety and quality.
The Forks Community Hospital Patient & Family Advisory Council will consist of a mix of community members and staff, who will come together to enhance the overall patient experience at FCH.
- Participate in discussion groups.
- Give input on new forms, brochures, and processes here at Forks Community Hospital.
- Keep updated on any upcoming projects and give feedback from a patient perspective.
- Review educational materials and provide feedback.
- Are encouraged to tell their story.
- Meet for 1 – 2 hours every 3rd Tuesday of the month from 2 – 4 pm in person or via Zoom.
A patient and family advisor is:
- Someone who wants to make a difference in patient care at Forks Community Hospital.
- A person that can give constructive feedback based upon a patient perspective.
- Someone who is able to volunteer their time to work on projects.
- An individual that can meet one time monthly for approximately 2 hours.
- Anyone who has received care at Forks Community Hospital in the last 5 years is encouraged to join.
- All cultures are welcome to participate.
- Special; qualifications are not necessary. Your experience as a patient is all the expertise we need!
You may be a good match if you:
- Speak up and share suggestions.
- Work well with people who may have differing opinions.
- Listen to what others have to say.
- Can focus and keep a good attitude during discussions.
- Are able to talk about your experiences as a patient or family member.
- Keep any information you may hear private and confidential.
