Vaccination Clinic Scheduled for July 1, 2021 in Forks, Washington

Are you over the age of 12 and in need of a COVID-19 vaccine?
There will be a Pfizer COVID-19 vaccination clinic on July 1, 2021 from 12:30 pm to 4:30 pm at Peninsula College.
If you are age 12-17 you will need to:
- Bring a guardian to give vaccination consent
- Bring this form, completed by your legal guardian
- Have a legal guardian available by phone to give verbal consent.
The Health Department in Forks is offering the Moderna and Johnson and Johnson COVID-19 vaccine on Thursdays.
