2008-9 Forks Community Hospital (FCH) Scholarship Award Program

The FCH Scholarship Award Program has been developed to enhance Hospital District recruiting in critical staff shortage areas and to promote the pursuit of healthcare occupations.

This year we are offering scholarships of up to $5,000 and more per year to students pursuing careers in Nursing, Physical Therapy, Physical Therapy Assistant, Pharmacy, Pharmacy Technician, Social Work, Mental Health, Chemical Dependency and other healthcare occupations.

Eligibility

You must be currently enrolled, or anticipating enrollment in an approved program at the time of scholarship award. Approved programs will result in professional registration, certification or licensure. Students must demonstrate satisfactory program progress to remain eligible for FCH Scholarship Award funding.

Award Amount and Timing

The amount of FCH Scholarship award will vary depending on projected hospital need, academic program and individual merit. Type, length and expense of academic program are considered as well as individual factors including scholastic performance, financial need and ensuing FCH Service Agreement. Scholarship funds are paid at the beginning of the quarter/semester and may be used for any purpose at the recipient’s discretion.

FCH Service Agreement

Upon satisfactory completion of the academic program, recipients of FCH Scholarship awards are expected to provide 1-3 years of service with FCH depending on the occupation and amount of scholarship award. Recipients are relieved of this obligation if a position for which they are trained is not available.
