Long Term Care Facility

Forks Community Hospital's Long Term Care Facility serves the residents of western Clallam and Jefferson counties. The facility now accommodates 20 Residents The facility has been designed to furnish a comfortable, home-like atmosphere, while offering close medical supervision. The staff is dedicated to providing comprehensive care with a personal touch.

Spacious, semi-private rooms, some with patios, offer residents the opportunity to surround themselves with some of their own furnishings. Natural skylighting in the hallways and TV and visitor lounges brighten the environment. Family pets are welcome to visit.

A skilled, professional nursing staff provides individualized care, and should the need arise, the staff and facilities of the Hospital are immediately available. A registered dietitian furnishes individualized dietary counseling and assures that healthful food is presented in an appetizing manner. Meals are served in a bright and airy dining/day room. A medical social worker and mental health professionals are available to both residents and their families to ease the adjustment to this new life style.
