The social worker provides patient assessment activities for acute care and long term care patients in coordination with case management and nursing staff on both units. The social worker is responsible for providing staff education related to patient communications, psycho social assessments, and family support activities. This position facilitates family meetings, family resources and discharge planning.

QUALIFICATIONS: Education: Bachelor’s Degree in Social Sciences Required, MSW Preferred. Healthcare experience with emphasis on mental health services preferred. Gerontology education helpful.
